Our approach is rooted in a holistic understanding of each person’s unique needs. We prioritise the wellbeing and dignity of every person we support.

From the initial assessment to ongoing care planning, our dedicated team works consistently to ensure the safety, health, comfort and happiness of those we support. 

We maintain open lines of communication, actively involving clients and their families in the decision-making process.

Our Process

1. Initial enquiry or referral
Individuals needing support can be referred through direct enquiries from their family members, through funding authorities, or self-referral.
2. Pre-Assessment
This involves identifying the support needs of the person requiring care and understanding their accommodation requirements. Suitable options for accommodation can include single occupancy, shared accommodation, or outreach support.
3. Full Assessment
We will carry out a comprehensive assessment to determine the individual’s support needs, health requirements and any risks that might be associated with their care.
4. Support summary and proposal of support
Following agreement from the local authority or ICB and based on the assessment results, we will create a support summary that details our proposed support.
5. Planning phase
We will begin by outlining the structure of the support team and identifying the specific skills necessary to ensure the individual remains safe and comfortable in their environment. Additionally, we will collaborate with housing partners to locate and modify accommodations to meet the person’s unique needs.
6. Development of personal support plan
The personal support plan sets out goals that will help the people we support fulfil their potential. It is drawn up following collaboration with the individual, their family and healthcare professionals, and will be used as a framework for determining the support we will recommend.
7. Assembling the support team
Introduction of the support team and observations. Observations play a pivotal role in this process to help us better understand person needs. This understanding allows us to implement personalised strategies that ease transitions effectively
8. Assisting with new tenancies
We work with dedicated housing partners to make tenancy arrangements as straightforward as possible. As well as finding the right accommodation, we help people to settle in and manage everything that comes with being a tenant.
9. Support starts
Once everything is in place and approved, we can begin our support services. This might be in a new home setting or outreach support if the individual is being cared for in the community or their family home.
10. Regular reviews
After six weeks, we will carry out an initial review to assess the effectiveness of the support provided. We will make adjustments accordingly and then continue to monitor progress on an ongoing basis. We also conduct annual reviews where we do a formal assessment of the support in place and suggest new strategies if necessary.
