The assessment process

We will meet with the individual to assess what level of support is required to ensure we are continuing to support individuals to reach their full potential. Starting with a pre-assessment, which identifies what makes each individual unique and provide an understanding of any environmental adaptions maybe required this could include accommodation and staffing. We then follow up with a comprehensive assessment to gain a more holistic understanding of the individual’s support needs, health requirements and potential challenges they may face.

The assessments form part of our 10-step Process as detailed in Our Approach.

Who can make a referral?

Referrals come to us from a wide variety of sources, including local authorities, the NHS, placement breakdowns, Special Education Needs schools, residential colleges, community support units and individuals looking to leave the family home for the first time. 

Referrals can be made by a social worker, care manager or the individual’s parents, although the care manager acting on behalf of that individual must be consulted before an assessment can take place.

How long do referrals take?

We aim to process referrals as quickly as possible but it is more important to us that we take the time to put the right support in place as this results in better outcomes for all involved. Our 10-step Process, as detailed in Our Approach, usually takes between 6-8 weeks to complete, although we can be dynamic and flexible depending on capacity. 

That means we can sometimes accept emergency referrals or fast track referrals with restricted timescales. Please speak to our team to find out more about emergency referrals.
