Transition Support – Steppingstone Model
At Brighter Living we strive to provide exceptional care and support to children and young adults who may be transitioning between children’s placements and adult services or leaving home for the first time and looking to move on into their own tenancy.
Our outreach support workers are on hand to provide as little or as intensive support as the person may need, typically within the family home or supporting people to access the community and build on independent living skills. Our skilled support workers are trained to provide support in a therapeutic, love led model which enables children to feel safe, empowered and allowing them to flourish.
